Nine Landmark Breast Cancer Papers and What They Show

These nine landmark breast cancer papers form a reading path through HER2 biology, inherited susceptibility, molecular classification, and clinical translation. Read them alongside a breast cancer research timeline to see how different kinds of evidence changed the questions scientists could ask. This is a selected historical guide, not a ranking or a review of every current treatment.

1 HER2 amplification and clinical behavior in 1987

Slamon and colleagues linked HER-2/neu amplification with relapse and survival in a historical cohort of primary breast cancers. Read this paper for the transition from an oncogene observed in model systems to a clinically relevant breast cancer feature. Slamon et al., Science

What it does not prove: association alone does not prove that blocking HER2 will improve outcomes. That required functional experiments and clinical trials.

2 Mapping familial breast cancer in 1990

Hall, King, and colleagues used linkage analysis to localize a major early-onset familial breast cancer susceptibility locus to chromosome 17q21. Read it for an example of how family data and genetic markers can establish the existence and location of a disease-susceptibility gene before the gene itself is known. Hall et al., Science

3 Identifying BRCA1 in 1994

Miki and colleagues reported a strong candidate for BRCA1 through positional cloning and identified likely predisposing mutations in linked families. Read it as the next step after mapping: from a chromosomal region to a defined gene that can be sequenced and studied mechanistically. Miki et al., Science

4 Molecular portraits of breast tumors in 2000

Perou and colleagues used gene-expression profiling to show that breast tumors could be organized into reproducible molecular patterns. Read it for the conceptual shift from one anatomical disease toward multiple molecularly distinct forms of breast cancer. Perou et al., Nature

5 Trastuzumab in early HER2 positive disease in 2005

The HERA trial tested trastuzumab after chemotherapy in HER2-positive early breast cancer and demonstrated improved disease-free survival. Read it for the clinical translation step: a biomarker-defined target is meaningful only when an intervention improves outcomes in the corresponding population. Piccart-Gebhart et al., New England Journal of Medicine

6 TCGA molecular portraits in 2012

The Cancer Genome Atlas integrated multiple data types—DNA copy number, methylation, exome sequencing, mRNA, microRNA, and protein—to characterize breast tumor classes. Read it for the power and limits of multi-omic classification: broad subtypes become clearer, but substantial heterogeneity remains within them. TCGA Network, Nature

7 OlympiAD and PARP inhibition in 2017

OlympiAD compared olaparib with physician’s-choice chemotherapy in patients with germline BRCA-mutated, HER2-negative metastatic breast cancer. The trial reported longer progression-free survival with olaparib. Read it for the translation of DNA-repair biology into a phase 3 strategy; the result does not establish benefit for every BRCA finding or every breast cancer setting. Robson et al. 2017

8 KEYNOTE 522 in early triple negative disease in 2020

The 2020 KEYNOTE-522 report studied pembrolizumab plus chemotherapy before surgery in previously untreated stage II or III triple-negative breast cancer. The trial also included postoperative pembrolizumab or placebo. The first interim analysis showed a higher pathological complete response rate; later follow-up addressed survival outcomes. Read the 2020 paper for its reported endpoint rather than treating it as the complete follow-up record. Schmid et al. 2020

9 DESTINY Breast04 and HER2 low disease in 2022

DESTINY-Breast04 compared trastuzumab deruxtecan with physician’s-choice chemotherapy in previously treated, HER2-low metastatic breast cancer. HER2-low was defined as IHC 1+ or IHC 2+ with negative in situ hybridization. The trial reported longer progression-free and overall survival, while also documenting important risks, including interstitial lung disease. Read it for a change in the treatment relevance of lower HER2 expression, not as evidence that every HER2-negative tumor is equally responsive. Modi et al. 2022

How to read and cite a landmark paper

Record the population or model, biomarker definition, comparison, endpoint, and follow-up period. Then ask what the study could not establish. Use research databases to locate follow-up publications and related datasets, and distinguish the original result from later clinical or regulatory developments.
